Piloted means the roster clears it while you do something else with your evening. On a plain run, loot follows the group rules. With the loot-priority option, eligible boosters pass matching drops to you instead of rolling against you — the difference between "we cleared the raid" and "I walked out with the trinket I came for". It costs a percentage of the run, and it is the option worth paying for when one slot upgrade is the entire reason you are here.
