Looks only. Whichever mode you pick, you are on the same realm, the same rules and the same 60 cap as everyone else — it is purely how the game renders characters and terrain on your screen. The toggle is a client setting, so two people in the same group can run different modes without noticing anything on the mechanics side.
What it does not do, based on what has been shown, is touch UI scale, addon behaviour or combat text. Those remain separate settings.
Will spell effects look different from Classic?
Nobody outside Blizzard knows yet. All nine classes and their specialisations are being revamped — Paladins get a tanking seal and a full Retribution kit, talents drop the +1–2% fillers — and new abilities need some visual to go with them, so it would be surprising if nothing changed. But "would be surprising" is not a fact. Until Blizzard shows spell visuals, or the beta reveals them from September 17, the honest answer is: unknown.
